US Club Soccer Player Registration Cheat Sheet

US Club Soccer Registration
All participants in the San Diego Premier League must have a US Club Soccer player/staff card.
 
If your Club is not presently a member of US Club Soccer, you will need to join
US Club Soccer as a San Diego Premier "League Team".  Clubs with 3 or more teams must register directly with US Club Soccer.
 
Below are the forms we will need completed and submitted to SDPL, along with an original birth document and the appropriate fees, so that we can issue you your player and staff cards.

#R002: US Club Soccer Player Registration & Medical Treatment Authorization Form - $14/per player. This is the form which is required of all players who are registering through their club/team with US Club Soccer. As an alternative to receiving this form from your club, players or parents may download the form from this site, complete it, and submit it to your Team Manager.

#R004: US Club Soccer Staff Risk Assesment Form $8/per staff member - Have each coach and administrator who work with players complete this risk management form. Send the original to us and keep a copy for your records. This is the only paperwork necessary to submit for a staff member.
